Greetings from Academic Advisement!
To better assist you in your academic career, we have provided the following student forms, which can be downloaded at your convenience. Not sure which Transfer Credit form you should use? Check the flow chart to be sure!
Forms
- The Advisement Department will NOT accept any forms that have missing information such as an ID number or Social Security number, advisor signature.
- Students must completely fill out all forms.
- Students are encouraged to keep a copy of all documents for their records.
- Students will receive their copy once the form has been received or processed unless the form does not indicate a copy is provided for the student.
- Academic Advisement is not permitted to sign for any Departments for any reason/circumstance.

Academic Program Listing Change Form (For Major, Minor or Emphasis)

Why
|
If a student decides to change, add or drop a major, minor and/or emphasis
|
|
When
|
Major: A student may change their major at any time so long as they have declared a major prior to the end of their sophomore year.
Minor: A minor may be added at any time prior to the last semester of their junior year.
Emphasis: The student may file this form at any time.
|
|
How
|
- The student talks with a Department Chair in the desired department.
- The student completes form with the Department Chair.
- The Department Chair signs the form (possibly assigning a new or additional advisor).
- The student’s current advisor’s signature is also required.
- The student returns the form to the Academic Advisement Office.
The student will receive a stamped copy of the form that Advisement has received it for processing.

Helpful Info
|
- Students must fulfill the requirements of their major according to the current catalog the term they declare their major.
- Most minors require 18 units, while others require more.
- Business majors are required to have an emphasis in order to graduate.
- Business majors must have an emphasis prior to adding a major
- Sociology emphases are optional.
- If a student declares their major under a different catalog than the one they started MSMC with, the general studies requirements will be fulfilled according to the catalog with which they entered.

Entrepreneur Certification Form

|
If a student wishes to add an Entrepreneur Certificate in the area of their pre-existing major.

When

How
|
- The student talks to their advisor.
- The student completes the top part of the form including their signature and their advisor's signature.
- The student requests that the Department Chair fill out the bottom portion, including their signature.
- The student files the form with the Academic Advisement Office.

Study Abroad Application
|
|

Why
|
If a student wishes to apply to study for a semester abroad.
|

When
|
By the dates indicated on the form.
|
|
How
|
- The student talks to the Study Abroad advisor
- The student completes the form including the signatures of the student, the advisor, Financial Aid, Business Office, and the Baccalaureate Dean.
- The student files the form with the Academic Advisement Office.

Transfer Credit Evaluation Contestation Form
|

|
If a student wishes to challenge their Transfer Credit Evaluation

When
|
During their first semester at MSMC
|
|
How
|
- Student completes the form including their signature and their advisor's signature.
- The student files the form with the Academic Advisement Office.
|
|
Helpful Info
|
- Submit any supporting documentation (course descriptions, syllabi, etc.) with this form.
- Separate supporting documents will not be accepted under any circumstances.
- Per the MSMC catalog, this form should be submitted during your first semester at the Mount. If you are contesting courses after your first semester this form must be accompanied by an approved Academic Petition exempting you from the one semester policy.
- Major Coursework requires the approval of the student's Department Chair; Academic Advisement cannot make decisions about courses required for a student's major or minor.

Transfer of Credit Clearance Forms:
General Studies, Major/Minor, Study Abroad
|

Why
|
If a student plans to take a course at another college, or if a student plans to study away at another CSJ college or with the AIFS Program (Study Abroad/Away).
|
|
|
When
|
The form must be completed by the dates indicated on the form
|
|
|
How
|
- The student decides on the appropriate course(s) to take in conjunction with their advisor
- The student completes the form
- The advisor signs the form
- The Department Chair and Academic Advisement sign the form if necessary
- The student then files the form in the Academic Advisement Office
- The form is then sent to the Associate Academic Dean for final approval
|
|
|
Upon completion of the course(s), the student requests their transcripts to be sent to the MSMC Registrar’s Office.
|
|
Washington Semester Application
|
|

Why
|
If a student wishes to apply to the Washington Internship Program in D.C.
|
|
When
|
By the dates indicated on the form
|
|
How
|
- The student completes the top portion of the form.
- The student then has the form completed by the nominator
- The student completes the form with the signatures of the advisor, Office of Student Financing, the Business Office, and the Baccalaureate Dean.
- The student files the form with the Academic Advisement Office.
