More Forms Explained

Academic Petition

Who?

Student initiated.

Where?

Registrar’s Office & Academic Advisement.

Why?

A student who wishes to extend an Incomplete, enroll in more than 18 units, have a general studies requirement waived, or some other reason not covered by an existing form.

When?

The deadline varies depending on the reason for the petition.

How?

  1. The student completes the form, including a detailed reason of why she/he is filing a petition.
  2. The advisor reviews and signs.
  3. If the student is asking for an extension of an incomplete, then the instructor and the Department Chair must sign.
  4. The form is filed in the Registrar's Office.
  5. When the Academic Dean has approved it, a copy will be sent to the student.

 

Add/Drop Form

Who?

Student initiated.

Where?

Registrar's Office.

Why?

After a student has registered for classes they may decide to make a change, or there may be a change in the list of classes necessitating a change for the student.

When?

There are many deadlines that impact this form.  Be sure to consult the academic calendar to determine what signatures are needed, and what the repercussions are.  The earliest this form is filed, the better.

How?

Before Class Begins:

  1. The student completes the form after consulting with their advisor or Academic Advisement.
  2. The advisor reviews and signs the form.
  3. The student files the form at the Registrar's Office.

First week of class:

  1. The student completes the form after consulting with their advisor.
  2. The advisor reviews and signs the form.
  3. The instructor signs (only if the student is adding a class).
  4. The student files the form at the Registrar's Office.

After the first week of class, before the last day to withdraw:

  1. The student completes the form after consulting with their advisor or Academic Advisement.
  2. The advisor reviews and signs.
  3. Instructor signs (only if applicable).
  4. An $11 fee applies
  5. The student files the form at the Registrar's Office.
  6. If the change results in less than 12 units or more than 18, Financial Aid and the Business Office must also sign the form. 
  7. If more than 18 units, the student must petition to the Associate Academic Vice President, except where the student is an Accelerated Nursing or Music Student.

Helpful Info:

  • Students are generally not permitted to add a class after the first two weeks of class.
  • An instructor's signature is NOT required to drop a class.
  • Advisement will NOT sign for instructors for any reason.

 

Cross-Program Registration Form

Who?

Student initiated.

Where?

Registrar's Office.

Why?

A student may register for a course in another MSMC program to enrich and supplement the MSMC program in which they are enrolled.

 

 

When?

A student may cross-program register at any time so long as the MSMC deadlines are met.

How?

  1. The student meets with their advisor.
  2. The student completes the form including the signatures of their advisor and the academic dean.
  3. The student files the form with the Registrar's Office.
  4. A copy of the form will be returned to the student as soon as the form is processed.

Helpful Info:

  • If the student chooses to drop the cross registered course, an add/drop form is required and filed in the Registrar's Office.

 

Leave of Absence

Who?

Student initiated.

Where?

Registrar's Office.

Why?

If a student decides to leave the institution for a semester, she/he should file this form.  The student is expected to return to MSMC the following semester, and is expected to contact the Baccalaureate Dean and their advisor.

When?

The form should be filled out as soon as the student decides to leave school.  The date the form is turned in may affect any refund that is offered to the student.  Please refer the student to the Business Office.

How?

  1. The student completes the first portion of the form.
  2. The student obtains the signatures of their advisor, or the Academic Advisement Office, the Office of Student Financing, the Business Office, and the Baccalaureate Dean.
  3. The form is then filed in the Registrar's Office.

 

Placement Test Application

Who?

Student initiated.

Where?

Academic Advisement (Doheny students:  Please refer to the Learning Resource Center for all your Placement Testing needs.)

Why?

If a student wishes to place in a higher language course, or to fulfill the General Studies Language requirement.

When?

The student may file the application at any time.

How?

  1. The student completes the form.
  2. The student files the form with the Academic Advisement Office.

Helpful Info:

  • The student needs to make an appointment to take the placement test when they file the form.
  • A copy of the form will be sent to the student once the form has been processed.

 

Program Change Application

Who?

Student initiated.

Where?

Registrar's Office.

Why?

If a student wishes to change MSMC Programs.

When?

By the published deadline given by the Intercampus Transfer Programs Office.

How?

  1. The student meets with their advisor.
  2. The student completes the form with the advisor signature and Testing Coordinator's signature (if necessary).
  3. The student files the form in the Intercampus Transfer Programs Office.

 

Washington Internship Application

Who?

Student initiated.

Where?

Academic Advisement (Chalon).

When?

By the dates indicated on the form.

Why?

If a student wishes to apply to the Washington Internship Program in D.C.

How?

  1. The Student completes the top portion of the form.
  2. The student then has the form completed by the nominator.
  3. The student completes the form with the signatures of the advisor, Office of Student Financing, the Business Office, and the Baccalaureate Dean.
  4. The student files the form in the Academic Advisement Office.

Helpful Info:

The form should be filed along with the Transfer of Credit Clearance form for the Semester in Washington Program.

 

Washington Semester Program

Who?

Student initiated.

Where?

Academic Advisement (Chalon).

When?

By the dates specified by the Washington Semester Program.

Why?

If a student wishes to apply to the Washington Semester Program.

How?

  1. The student meets with an advisor from Academic Advisement, and together they complete the form.
  2. The student encloses or has the MSMC Registrar's Office send an official copy of the student's transcripts.
  3. The student then sends the form to the Dean at American University.

Helpful Info:

  • If the student has more questions, she/he should speak to their advisor.

 

Withdrawal

Who?

Student initiated.

Where?

Registrar's Office.

When?

As soon as the student makes the decision.  Students should be aware of college posted deadlines, regarding refunds.

Why?

If a student wishes to withdraw from the college.

How?

  1. The student talks with an advisor.
  2. The student completes the form, including the signatures of the student's advisor, or the Academic Advisement Office, Office of Student Financing, the Business Office, and the Baccalaureate Dean.
  3. The form is then filed in the Registrar's Office once all the signatures are obtained.

Helpful Info:

  • Resident students must also follow the procedures of the Residence Life Office.
  • The students must sign the form; otherwise the withdrawal is not valid.
  • If a student wishes to return to MSMC they must re-apply through the Admissions Office.
