Health Services Confidentiality Policy

Confidentiality is important to us and is maintained for all services provided to students. A student's medical record is only released with the student's request and written permission or by judicial subpoena. 

Information cannot and will not be released to parents without prior consent from the student, unless the student is less than 18 years of age. We consider each student to be an "emerging adult", capable of making independent educated decisions.

Medical records are kept for seven years after graduation and are then destroyed. If you would like to request a copy of your medical records from MSMC Student Health services, you may fax or mail the Request for Medical Records [form] to the campus you attended. Please note: Copies of medical records will take up to 7-10 business days.
