Health Requirements
Health information should be submitted by August 1st for the Fall semester. The health forms are available for pick up in the MSMC Student Health Service center or you may download the forms from this website. All resident MSMC students must have completed health forms on record in the MSMC Student Health center prior to moving into the dormitories.

Where should I go for my physical examination and required immunizations?
Returning students may schedule an appointment with Student Health Services for all health compliance and residency needs. Incoming students should make appointments with their own healthcare provider or may seek services at the following locations:
