Health Requirements

For all Traditional Undergraduate Students

1) Copy of official historical vaccine record to include:

• (PPD) TB Skin Test within the past 12 months or Chest X-Ray within the past two years for those who have a history of a positive PPD
• Tdap Vaccine
• MMR Vaccine #1, #2
• Hepatitis B Vaccine #1, #2, #3
• Residents Only: Meningococcal Vaccine or Meningococcal Vaccine Waiver   

2) Download and fill out the following:

• Initial Health History [form]
• Residents only: Meningococcal Vaccine Waiver [form] for those who do not intend to receive the vaccine

3) All required immunizations are to be submitted through our Student Health Immunization Self-Reporting Portal no later than AUGUST 1st for Fall Admission. For detailed instructions on how to submit this information: [CLICK HERE]

Highly recommended vaccines: Hepatitis A, HPV (Gardasil), Varicella (Chickenpox), Influenza

   

Additional requirements for Traditional BSN, ABSN, ADN, and DPT Students

Students officially accepted and enrolled in these programs (not Pre-Nursing or Pre-Health) should refer to information packets received annually from their divisions for additional requirements necessary for clinical rotation compliance.
