Medical Records and Confidentiality  

Confidentiality is important to us and is maintained for all services provided to students. Information cannot and will not be released to parents without prior consent from the student, unless the student is less than 18 years of age. We consider each student to be an "emerging adult," capable of making independent educated decisions.

A student's medical record is only released with the student's request and written permission or by judicial subpoena. Medical records for services provided within Student Health Services are kept for a period of seven years and then destroyed.


If you submitted your vaccine records directly to Student Health Services you may now print them out from your own computer.

2. FIRST TIME USERS must Register for an account on this site. This might take 2-3 attempts if you are using certain browsers, including Internet Explorer. Add a zero at the beginning of your student identification number if it does not contain seven numbers.

3. Once you are in the site, click on IMMUNIZATIONS, then click View History. Your record should appear, simply hit the printer icon. 


If you would like to request a paper copy of your medical records from Student Health Services:

Please fax or mail the completed CONSENT FOR RELEASE OF MEDICAL INFORMATION [form] to the campus you attended. Copies of medical records will take up to 7-10 business days.
