Print a copy of your Immunization Record

If you submitted your vaccine records directly to Student Health Services you may now print them out from your own computer.

1. Click this link https://msmc.medicatconnect.com/index.aspx

2. FIRST TIME USERS must Register for an account on this site. This might take 2-3 attempts if you are using certain browsers, including Internet Explorer. Add a zero at the beginning of your student identification number if it does not contain seven numbers.

3. Once you are in the site, click on IMMUNIZATIONS, now click View History. Your record should appear, simply hit the printer icon.
