Change of Grade Form

Who: Instructor initiated
Where: Registrar’s Office
Why: To change a grade for a student in a particular course
When: When a grade is mistakenly assigned by instructor, clerical error, removal of Incomplete or In-Process.
How: Change of Grade forms should be filled out and signed by instructor before the 30 day deadline. Change of Incomplete by the last day of the next semester. 

NOTE: Forms are always given to instructors ONLY.
