Why:	To verify current enrollment of term or previous terms and graduation date, major(s), minor(s) and honors.
When:	This form should be filled out for the purpose of car insurance, health insurance, internships, and employment.
How:	Fill out form completely Attach any enrollment form or return envelope. Indicate whether verification is to be mailed by MSMC or picked up by the student.
