Clinical Clearance - Requirements

All items must be submitted prior to the start of clinical in order to be cleared to begin. You are required to keep copies of all required documents and be prepared to show copies requested by your clinical instructor.

The following are required to be updated for each clinical rotation:

Health Requirements

  • Complete Physical Exam (must include complete blood count/Urinalysis/Vision Exam)
  • PPD or Chest X-Ray if PPD is positive

Other Requirements

  • CPR Certification
  • Malpractice Insurance
  • First Aid (Senior Level Only)
  • Driver License (Senior Level Only)
  • Auto Insurance (Senior Level Only)

The following are required to be completed once

  • Hepatitis B series (1, 2, & 3)
  • Hepatitis B serum titer (Hepatitis B surface antibody positive)
  • Rubella (record of immunization or positive serum titers)
  • Rubeola (record of immunization or positive serum titers)
  • Varicella Titer (record of immunization or positive serum titers)
  • Tetanus (TD)
  • Background Check

Students who have exceptions to any of the above are to contact the B.S.N. coordinator of clinical clearance. Note: facilities may not accept students who have not completed all clinical requirements.
